Estrogen and progesterone, the two main hormones behind your cycle, don't just control your period.
They also shape your vaginal microbiome, the community of bacteria living in your vagina. Every month, on repeat, for as long as you are cycling.
The Basic Mechanism
Estrogen (the hormone that rises through most of your cycle) tells your vaginal walls to store more glycogen, a type of sugar.
That glycogen feeds Lactobacillus, the main "good bacteria" in a healthy vagina. Lactobacillus turns it into lactic acid, which keeps your vaginal pH (a measure of acidity) low. That acidity makes it harder for unwanted bacteria to grow.
More estrogen tends to mean more Lactobacillus. That's a big reason the childbearing years, when estrogen runs higher, are linked to a more stable vaginal environment than the years before puberty or after menopause. Before puberty and after menopause, estrogen is low, glycogen drops, and Lactobacillus tends to drop right along with it. This is normal. It is just biology on a different setting.
Your Cycle Isn't as Steady as It Feels
Your vaginal environment is not the same on day 3 as it is on day 20. It moves through a rhythm.
During your period: Blood temporarily raises vaginal pH and iron levels, and low estrogen means less glycogen is being made. Research shows this often brings a small, short-term dip in Lactobacillus and a short-term rise in overall bacterial diversity. Iron from the blood can also give a boost to bacteria like Gardnerella, one of the species tied to bacterial vaginosis.
Follicular phase (right after your period): Estrogen climbs, glycogen builds back up, and Lactobacillus tends to bounce back.
Ovulation: Around mid-cycle, increased hydration in the vaginal lining lowers viscosity, which is one reason discharge often looks and feels different around this time. pH tends to move toward more neutral.
Luteal phase (after ovulation): Progesterone, the hormone that helps prepare the body for possible pregnancy, joins in and helps release stored glycogen, continuing to feed Lactobacillus. The lining also becomes a bit thicker and less hydrated, which some researchers think may help trap unwanted organisms before they get further in.
Noticing changes in discharge through your cycle isn't automatically a red flag. Some fluctuation seems to be a normal hormonal rhythm, not a sign something is wrong.
Beyond a Single Cycle: Pregnancy and Menopause
The same estrogen-glycogen-Lactobacillus pattern shows up at other points in life too.
During pregnancy, estrogen rises higher than it does in a normal cycle, and Lactobacillus tends to rise with it. This is thought to be one of the body's ways of protecting both mother and baby. After delivery, estrogen drops fast, and Lactobacillus tends to dip for a while before things settle back into a normal rhythm.
At menopause, estrogen declines for good. Glycogen drops, the vaginal lining thins, and Lactobacillus tends to decline too, while other bacteria like Gardnerella and yeast species like Candida can become more common. This is a well documented, expected shift, not a personal failure of hygiene or diet.
What About Birth Control?
Hormonal contraceptives adjust your natural hormone levels on purpose. Some research suggests this may also shift Lactobacillus levels, though it varies by method and study.
The research so far points to method mattering. Combined oral contraceptives, which contain both estrogen and progestin, have generally been linked to steadier, more Lactobacillus-friendly conditions, likely because of the added estrogen. Progestin-only methods, like the hormonal IUD, have shown more mixed results in different studies, and one longitudinal study found a locally released progestin device was linked to some loss of Lactobacillus species over time. This isn't a reason to avoid hormonal birth control. It is just good to know that anything changing your hormone levels can also touch your vaginal environment, and that this can look different from one method to the next.
Diet and Exercise Play a Role Here Too
Your cycle is not the only daily rhythm influencing this system. One study tracking young women day by day found that a vegetarian diet and intense exercise were both linked to lower Lactobacillus levels and more bacterial diversity, alongside the expected hormone-driven shifts. Researchers are not certain why yet, but it is a reminder that your vaginal microbiome responds to more than hormones alone. Energy intake, diet pattern, and exercise load may all be feeding into the same system.
